A decade ago, cell and gene therapy was still considered one of the riskiest areas in biotechnology. Manufacturing was complex, patient access was limited, and commercial success remained uncertain. Today, Novartis has become one of the companies leading the transformation of advanced therapies from experimental science into a large-scale commercial business.
The company’s gene therapy portfolio, led by Zolgensma and Kymriah, has helped Novartis build one of the strongest positions in the global cell and gene therapy industry. In the first nine months of 2025 alone, Zolgensma generated approximately $925 million in global sales, making it one of the highest-selling gene therapies in the world.
But the bigger story is not only about drug sales. It is about how Novartis is building the infrastructure required to dominate the next generation of medicine.
From Rare Disease Therapy to Billion-Dollar Commercialization
Novartis entered the gene therapy market aggressively through its focus on rare diseases and oncology.
Its spinal muscular atrophy treatment, Zolgensma, became one of the industry’s biggest breakthroughs because of its one-time treatment approach. Instead of lifelong therapy, the treatment targets the genetic root cause of the disease.
The therapy quickly became one of the most expensive medicines globally, priced around $2.5 million per patient. Despite the high cost, demand continued growing due to the therapy’s potential long-term impact for children suffering from spinal muscular atrophy.
At the same time, Novartis strengthened its oncology cell therapy business through Kymriah, one of the first FDA-approved CAR-T therapies for blood cancers. CAR-T therapies work by engineering a patient’s own immune cells to attack cancer, making manufacturing significantly more complex than traditional pharmaceutical production.

Manufacturing Became Novartis’ Biggest Competitive Advantage
One of the biggest challenges in cell and gene therapy is manufacturing scale.
Unlike traditional medicines that can be mass-produced relatively easily, gene therapies require highly specialized production systems involving viral vectors, sterile manufacturing environments, cold-chain logistics, and patient-specific processing.
Novartis recognized early that manufacturing capacity would become one of the industry’s biggest bottlenecks.
That is why the company announced a massive $23 billion investment into U.S. manufacturing and research operations. The expansion includes seven new facilities and additional investment across ten total sites.
The company’s strategy reflects a broader shift happening across the pharmaceutical industry. Governments are increasingly pushing companies to manufacture critical therapies domestically, especially in the United States.
For Novartis, manufacturing is no longer only a support function. It has become a strategic growth engine.
FDA Approvals Helped Accelerate Growth
Regulatory momentum has also supported Novartis’ expansion.
The U.S. Food and Drug Administration has accelerated approvals across the cell and gene therapy sector in recent years. Novartis benefited directly through approvals tied to both oncology and rare disease treatments.
In 2025, the FDA also removed REMS restrictions for several CAR-T therapies, including Kymriah. The removal simplified treatment access for hospitals and reduced administrative barriers for providers.
This regulatory shift is expected to support broader adoption of CAR-T therapies in the coming years.
Global Cell & Gene Therapy Expansion
| Area | Novartis Position |
|---|---|
| CAR-T Therapy | Among earliest FDA-approved players |
| Rare Disease Gene Therapy | Global leader through Zolgensma |
| U.S. Manufacturing | Multi-billion-dollar infrastructure expansion |
| Advanced Therapy Pricing | Among highest-priced therapies globally |
| Oncology Cell Therapy | Expanding commercial footprint |
| Gene Therapy Commercialization | One of the largest globally |
The Industry Is Moving Beyond Research
The cell and gene therapy industry is now entering a new phase where success depends on more than scientific discovery.
Companies must now manage:
- Manufacturing scale
- Hospital treatment networks
- Cryogenic logistics
- Regulatory approvals
- Reimbursement systems
- Global supply chains
Novartis has positioned itself at the center of this transition.
Its strategy shows that the future of advanced medicine will likely belong to companies capable of combining scientific innovation with industrial-scale manufacturing and global commercialization.
